Why BMI has limitations for South Asians
A 2004 WHO Expert Consultation found that South Asians have significantly higher percentage body fat, visceral adiposity, and metabolic risk at the same BMI compared to Caucasian populations. At BMI 23–24.9 (overweight by Asian cutoffs but normal by WHO standard), Indian adults show markedly higher rates of insulin resistance, dyslipidemia, and hypertension. Use BMI as a screening tool, not a diagnostic one — waist circumference (>90 cm for men, >80 cm for women in South Asians) is a better predictor of cardiovascular risk.
How to measure height accurately in cm at home
For the most accurate BMI reading: stand barefoot on a hard floor against a flat wall. Place a book flat on your head perpendicular to the wall. Mark the wall where the book meets it. Measure from the floor to the mark. Morning height is typically 1–2 cm more than evening height because spinal discs compress during the day — for consistency, measure at the same time of day. Use the same measuring tape or stadiometer each time you re-check your BMI.
BMI for athletes and muscular individuals: why it fails
BMI was designed for population studies, not individual fitness assessment. A muscular athlete with low body fat may show an "overweight" or "obese" BMI because muscle is denser than fat. A 1.80m bodybuilder weighing 95 kg: BMI = 95 ÷ (1.80²) = 29.3 — "overweight" by any standard. But body fat may be 8–12%. BMI is most accurate for sedentary individuals with average muscle mass. BMI tracking over time: what changes mean
A single BMI reading is less useful than a trend. Track BMI monthly during a weight management programme. A decrease of 0.5–1 BMI point per month is healthy and sustainable. Rapid decreases (2+ BMI points per month) often indicate muscle loss, not just fat loss. If BMI is decreasing but waist circumference is not reducing, the loss may be lean mass — increase protein intake and add resistance training. Use BMI alongside waist circumference (target: below 80 cm for Indian women, below 90 cm for Indian men) for a complete metabolic picture.
BMI by age and gender for Indian adults
While the BMI formula itself does not change with age, health risk thresholds vary. Older adults may carry slightly more body fat at the same BMI without additional metabolic risk. The Asian-specific cutoffs below apply to Indian adults:

For Indian women, the overweight threshold (BMI ≥23) is associated with significantly elevated risk of gestational diabetes and PCOD. How to measure height accurately in centimetres at home
Accurate height measurement is critical for a valid BMI. Follow these steps for the most reliable reading:
- Remove shoes, thick socks, and any hair accessories that add height.
- Stand barefoot on a hard, flat floor against a wall. Keep heels together, touching the wall.
- Stand straight with your back, shoulders, and buttocks touching the wall. Look straight ahead.
- Place a flat object (book or ruler) flat on top of your head, perpendicular to the wall. Mark where it meets the wall.
- Measure from the floor to the mark using a metal measuring tape (fabric tapes stretch).
- Measure at the same time of day: height varies 1–2 cm between morning and evening due to spinal disc compression.
When BMI in kg and cm is misleading: who should not rely on it
The BMI formula (kg ÷ m²) was designed for population studies, not individual assessment. It fails to distinguish muscle from fat, making it unreliable for:
- Athletes and strength trainers — a 90 kg bodybuilder at 175 cm has BMI 29.4 (obese) with 8–10% body fat. The BMI "overweight" flag is meaningless.
- Elderly adults (65+) — muscle mass declines with age (sarcopenia). An elderly person may show "normal" BMI but have dangerously low muscle mass and high fat percentage.
- Children and adolescents — use the CDC pediatric BMI percentile charts (age and sex adjusted), not adult cutoffs. Our standard BMI calculator is for adults only.
- Pregnant women — BMI is not meaningful during pregnancy. Use gestational weight gain guidelines from ICMR.
- Individuals with large muscle mass from manual labour — construction workers, farmers, and similar occupations often have elevated BMI with low body fat.

Waist-to-height ratio: a better predictor of metabolic risk for Indians
Waist-to-height ratio (WHtR) = waist circumference ÷ height (both in same unit). Target: below 0.5 for all ages and sexes. Example: waist 84 cm, height 170 cm: WHtR = 84 ÷ 170 = 0.494 (within healthy range). Research consistently shows WHtR is a better predictor of type 2 diabetes, cardiovascular disease, and metabolic syndrome in South Asians than BMI alone.
